Our commitment
We keep your health information private as required by Florida law (Chapter 491, Florida Statutes) and by federal law, including 42 CFR Part 2, which gives extra protection to substance use treatment records.
How we use your information
We use your information to provide your care, to schedule and bill for services, and to run our practice, such as keeping records and meeting licensing requirements.
When we may share information without your written consent
Only when the law allows or requires it, including:

- If you bring a legal or licensing complaint against us, limited to that proceeding.
Substance use records (42 CFR Part 2)
We will not disclose that you receive substance use services, or share your records, without your written consent except as Part 2 allows. Your records cannot be used to investigate or prosecute you, or in a civil, criminal, administrative or legislative proceeding against you, without your written consent or a court order that meets Part 2 requirements. Anyone who receives your records with your consent may not share them further except as Part 2 permits.
Your consent
Any other sharing requires your signed, written consent. You may revoke consent at any time in writing, except for action already taken.
